SAI engagement materials

Location: Northern Health and Social Care Trust

We are currently updating our information and approach to how we involve service users and carers in Serious Adverse Incident (SAI) processes. We want to make sure this work reflects what matters most to you and supports meaningful, inclusive involvement.

As part of this, we have developed:

  • An updated SAI information leaflet (attached)
  • A draft template to support the involvement of families in SAI reviews (attached)

We have also created a short survey that will be used to gather feedback for families around engagement & involvement in SAIs:
