We are currently updating our information and approach to how we involve service users and carers in Serious Adverse Incident (SAI) processes. We want to make sure this work reflects what matters most to you and supports meaningful, inclusive involvement.
As part of this, we have developed:
• An updated SAI information leaflet (attached)
• A draft template to support the involvement of families in SAI reviews (attached)
We have also created a short survey that will be used to gather feedback for families around engagement & involvement in SAIs:

We would really value your feedback on:
• How clear and helpful the information is
• Whether the proposed approach feels supportive and appropriate
• Any suggestions you may have to improve how we involve service users and families
Your feedback will help us improve how we communicate and involve people during what can be a very difficult time, and ensure our approach is as respectful, transparent and supportive as possible.
If you have any feedback on this information we would appreciate you sending through here by Tuesday 30 June.
Thank you for taking the time to support this work
