The WHSCT would like to invite Carer/Service User representatives to join the Personalisation Development Forum as your insight and experience would be invaluable.

Background

The personalisation agenda envisioned under ‘Transforming Your Care’ encourages staff to recognise people as individuals who have strengths and preferences, putting them at the centre of their own care and support by offering choice about how and when they are supported. This is facilitated through information, advocacy and advice so they can make informed decisions

As such, Self-Directed Support (SDS) was established as a regional approach in 2015. This was a new and transformational change process in the way social care support and services were delivered. Central to this is offering more flexibility, choice and control to individuals in directing their own social care support and empowering individuals to have informed choice about how support is provided to them, with a focus on working together to achieve individual outcomes.

The Personalisation Development Forum (formerly the SDS Planning Group) was established to support and oversee the Self-Directed Support process whilst also focussing on leadership and support to promote and drive the principles of personalisation of care for every individual in receipt of service in WHSCT.

What to expect

As a member of the Forum you would be involved in providing feedback on areas for improvement, sharing ideas and contributing to discussions around personalisation of social care delivery and Self Directed Support.

Details

Meetings are primarily held via MS Teams (online) however a physical location for the meetings can be arranged if required.   You will be invited to attend every 12 weeks from 2-3pm.  Meeting dates to be confirmed.
