Are you interested in improving Health and Social Care services for older people and people with long term conditions in your local area?
We are looking for Service User and Carer volunteers to join our Integrated Care Partnerships. There are currently 17 Partnerships active throughout Northern Ireland.
If you are an older person or are living with a long term chronic condition, then we would like you to get in touch to share your experiences. We also encourage those caring for older people or those with long-term conditions to also get in touch.
What do Integrated Care Partnerships do?:
Each Integrated Care Partnership is charged with improving services for their locality, looking at how care is delivered from prevention through to end of life. They will then make changes so that services are joined up and are delivering the right care, in the right place at the right time. Each Partnership Committee has about 13 members and brings together doctors, nurses, pharmacists, social workers, hospital specialists, other healthcare professionals, service users and carers; as well as the community and voluntary sector and representatives from local councils.
They currently focus on specific service areas:
1. Services for Frail older people
2. Stroke services
3. Respiratory services
4. Diabetes services
